  • Driver’s License numbers
  • Government – issued ID numbers
  • Financial information, such as bank card and bank account numbers
  • Medical Health Information
  • Health Insurance Information

Because of this failure to safeguard the sensitive personal information entrusted to them, anyone who has been a patient of OakBend Medical Center and had their information stolen is at risk of identity theft and financial losses.
